Patient Forms

To complete and send us the forms electronically, you will need to sign up for the Follow My Health portal.

See your email from Follow My Health when you make your appt.
Click on the "create a Follow My Health account"
Your email is your user ID.  You create the password.
Your access code is the patient's date of birth with two digit month, two digit day and four digit year
You will then need to complete the forms that are under the messages tab in the Follow My Health dashboard.

When you make an appointment either in the office or online, the email you use to register will be sent to Follow My Health, our online HIPAA compatible portal.  You will then receive an email from Follow My Health providing you with details on how to create an account.  You can use this portal to message the provider (non urgent messages only) and view your labs and imaging results and other records.
